The SRS(Stereotactic radiosurgery) and SBRT(Stereotactic body radiation therapy) given through this machine is called Novalis Tx radiosurgery or body radiotherapy respectively.

Q4 What is LINAC?


A4 LINAC is a short form of Linear Accelerator machine that delivers radiation in the form of high energy X rays to the targeted area or tumor anywhere in the body and destroys the tumor cells while sparing the surrounding healthy and critical tissues. The shape of the beam is such,so as to coincide with the shape of the tumor and deliver radiation precisely and with accuracy. The safety mechanism in the machine ensures that only the prescribed radiation dosage is released. It is most often used as External Beam radiation therapy in cancer patients. Radiosurgery can be carried out with Linear accelerator machines.


Linear accelerator machine is initially built as a  general purpose, conventional radiation delivery machine. However, in order to be specifically used for radiosurgery or IMRT(Intensity modulated radiation therapy), it requires changes in the form of addition of another tool or piece of machinery. These modifications or addition of specific equipment may be permanently fixed to the radiation couch and the machine is dedicated for higher level advanced radiation treatment therapy or can be temporary attached, which means it can be used for specific advanced radiosurgery treatments for sometime after adding the specific equipment and later after removal of those machine parts can be used as general purpose conventional radiation delivery machine.


Most often, the Linac machines with dedicated function and permanent additional equipment are preferred to be used, as the other Linac machines wherein additional equipment is added off and on cannot effectively deliver radiation dose with precision and accuracy as compared to the dedicated LINAC machines for advanced radiation treatments.


Q5 What is Radiosurgery?


A5. Radiosurgery is an advanced form of radiation treatment given in one to five sessions. It is an extremely precise delivery of the high dose of radiation to the tumor or the affected area with minimal exposure of radiation to the normal healthy surroundng tissues. Although the radiation dose given is less than the dose otherwise given during conventional radiation therapy, however, the tumor gets very high dose of radiation in a single session and the results procured are that comparable to that of surgery. It is indicated in the treatment of several types of cancer like the tumors in the brain, spine, lung, liver and prostate besides various non malignant disorders.


Radiosurgery can be carried out with Linear accelerator machines by coordinated efforts of a Neurosurgeon and a Radiation Oncologist. Radiosurgery, generally, is a one session procedure, the entire treatment session is completed in a single day that encompasses immobilization, planning, scanning and the actual procedure.


Radiosurgery usually delivers less dose of radiation in one time as compared to the total dose, aggregation of few small doses, that would be given in conventional radiation therapy. The tumor receives one time high dose radiation in case of radiosurgery and few smaller doses with radiation therapy given over a period of time. This is significant as more radiation will be released to the neighbouring healthy tissues when the patient undergoes few treatment sessions instead of one which will result in more side effects and also the tumor, the target area will receive less dose of radiation leading to poor tumor control and compromised outcome than with Radiosurgery.


Q6 How Steriotactic Radiosurgery is different from Fractionated Stereotactic Radiotherapy?


A6 When radiation treatment is given in a single session in high dose, it is referred to as Stereotactic Radiosurgery. However, when it is given in a series of sessions over a period of time , it is referred to as Fractionated Stereotactic Radiotherapy.

Unlike Radiosurgery, radiation is delivered in lesser amounts. Though, total dose of radiation given is similar in both. Fractionated Stereotactic radiotherapy permits the healthy tissue to recover from the effect of the radiation ahead of the next treatment session. Novalis Tx provides both Stereotactic Radiosurgery and Fractionated Stereotactic Radiotherapy. Q7 What types of equipments are used to carry out SRS (Stereotactic Radiosurgery)?


A7 There are generally three types of equipments in use to carry out SRS. They differ in terms of instruments and the source of radiation. They are:

    • ​Gamma Knife uses 192 or 201 beams of Gamma rays, highly focussed at the target area.
    • Suitable for treating intracranial lesions(abnormalities within the skull) that are small to medium sized.
  • Linac(Linear accelerator machine)
    • ​Deliver high energy X rays or photons
    • Manufactured by different companies by different names such as Novalis Tx, Cyberknife, X Knife
    • Can be used for SRS(Stereotactic Radiosurgery) in a single session for treating large tumors or for Fractionated SRT(Stereotactic Radiotherapy) over multiple sessions.
  • Proton Beam Radiosurgery or Heavy Charged particle Radiosurgery
    • ​Limited use in North America


Q8 Who supervises and carries out the SRS procedure?


A8 SRS treatment calls for a coordinated effort of several specialized medical professionals such as Radiation Oncologist, medical radiation physicist, radiologist, dosimetrist, radiation treatment nurse all performing their respective roles. Sometimes a Neurosurgeon also supervises the treatment in addition to the radiation oncologist. Their role includes but not limited to identifying the target area, determining the suitable radiation dose, finalizing the treatment plan and analyzing the results of radiosurgical procedures.


Radiologist interprets imaging that determines the target to be treated in the brain or the body. Medical Radiation physicist makes sure that the exact dose of the radiation is delivered. A special computer software is utilized to formulate an appropriate treatment plan by calculating the beam configuration and exposure in order to treat the target area with the prescribed dose of the radiation in the best possible way by the dosimetrist under the supervision of senior professionals.


Well trained Radiation therapist or Neurosurgeon and/or Radiation Oncologist may help in appropriate patient positioning and actually carrying out the procedure. The radiation treatment nurse assist the patient and the specialist before, during and after the procedure. He/she educates the patient, assesses and monitors the patient throughout.
