Q1 What is the structure of shoulder joint?

A1 Shoulder joint, like any other joint, is an area where two or more bones meet or join together. 3 bones join together to form shoulder joint, namely, Humerus (upper arm bone), Clavicle (collar bone) and Scapula (shoulder blade). Shoulder joint is a ball and socket joint in which ball shaped head of the upper arm bone fits into the socket shaped part of the shoulder blade. This socket is referred to as Glenoid.

 

Q2 How does our shoulder joint function smoothly?

A2 Normally, articular cartilage, a layer of tissue, covers the touching surfaces of the bones that rub against each other and keep these surfaces apart preventing their friction, during the movement of the shoulder joint. The articular cartilage smoothens the surface of bones and help them glide smoothly. Furthermore, there is a synovial membrane that covers the rest of the surface in the shoulder joint. It secretes small amounts of fluid which lubricates the joint and reduces the friction amongst bones. The neighbouring muscles and tendons provide the required stability and support to the shoulder joint for its efficient functioning.

 

Q3 What are the causes of painful and stiff shoulder joint?

A3 The conditions which cause painful and stiff shoulder joint are:

  • Osteoarthritis: One of the most common reason for shoulder replacement surgery, also referred to as Degenerative Joint Disease. The articular cartilage that cushions and protects the surface of the bones against rubbing each other, wears off. This leads to bare bone surface and their friction. It usually affects people over the age of 50 years. However, it may affect younger people too. With passing time, the condition worsens.

  • Rheumatoid Arthritis: This condition is characterized by inflamed and thickened synovial membrane that covers the inner surface of the shoulder joint capsule and produces viscous fluid that lubricates and nourish the joint. Its long standing inflammation leads to damaged cartilage, stiff and painful shoulder joint. It is a type of inflammatory arthritis.

  • Post Traumatic Arthritis As the name suggests, the arthritis or the joint inflammation that follows trauma (shoulder injury). Injury could be fracture of bones forming the shoulder joint, injury to the tendons or ligaments eventually leading to damaged articular cartilage. It results in painful shoulder joint with compromised function.

  • Rotator Cuff Tear Arthropathy: Long standing and untreated rotator cuff tear may eventually damages the articular cartilage and cause arthritis (inflammation of the joint).

  • Avascular Necrosis (Osteonecrosis) A(no) Vascular(blood supply) Necrosis (tissue death). Due to the lack of blood supply to the bone the bone tissue dies.

  • Severe Fractures

Q4 What is shoulder replacement surgery?

A4 Shoulder replacement surgery though a common surgery but not as common as hip or knee replacement surgery. It has a high success rate in terms of painfree and fully functional shoulder joint leading to an improved quality of life.

A5 There are three types of shoulder replacement surgery:

  1. Total Shoulder Replacement Surgery 

    • ​​Most common type

    • Ball shaped head of the upper arm bone (humerus) is replaced with a similar shape and size metal ball (implant/prosthesis) which is then affixed to the remaining bone and a plastic liner is affixed to the socket. Both the components of the shoulder joint get an implant.

    • Also known as Traditional Shoulder Replacement Surgery, Conventional Shoulder Replacement Surgery or Traditional Shoulder Arthroplasty

    • Most viable treatment option for patients with debilitating arthritis of the shoulder joint

    • Not recommended for people who want to indulge in a highly active life style or who have rotator cuff injury with damaged rotator cuff muscles.

  2. Partial Shoulder Replacement Surgery

    • ​​Only one component, the ball shaped head part of the humerus (upper arm bone) is replaced with a metal ball.

    • Nothing is done to the natural socket, the Glenoid.

    • Also referred to as Shoulder Hemiarthroplasty

    • A special and different version of Partial shoulder replacement surgery is Ream and Run in which natural socket is retained, no implant for socket, but it is smoothened and reshaped using special tools. It is also referred to as Hemiarthroplasty with Non Prosthetic Glenoid Arthroplasty.

  3. ​Reverse Shoulder Replacement Surgery

    • ​As the name suggests, there is reversal of the components from the usual pattern. It implies that the metal head is affixed to the shoulder bones which was originally a place of socket and the socket implant is affixed at the top of the humerus, the upper arm bone, which was originally ball shaped. There is reversal of ball and socket place in shoulder joint.

    • Also known as Reverse Total Shoulder Arthroplasty

    • It is recommended for people with damaged Rotator Cuff muscles. They are ineligible for traditional shoulder replacement surgery.

    • This surgery enables alteration of the center of rotation of the joint, thereby allowing other muscles to compensate for the loss of function of the rotator cuff muscles facilitating shoulder joint movement.

Q6 What is Shoulder Resurfacing?

A6 Shoulder resurfacing is also referred to as Resurfacing Hemiarthroplasty. In this kind of surgery, the damaged head of the upper arm bone, humerus, is covered with a smooth metal cap, to facilitate shoulder joint movement.

 

No replacement of the natural humerus head with a metal ball implant, no insertion of prosthetic metal stem in the humerus, just covering the damaged head with a smooth round cap.
